Just a pin head size die chip. It will slowly enlarge with more strikes.
Not uncommon on chromium plated dies.

That is because the last two numbers where hand punched into the die. With each die, the location would be slightly different. You could have a near, medium, far, high, low, depending on where the mint worker punched it. As there would have been approximately the same amount of coins from each strik...

Die clashes are fairly common on early 40’s fifty cent coins.

As you probably know, condition is the major factor in the value of a coin. A couple of grades can make a big difference in how much a coin is worth.
