Nickel 1920's Error, factory damage or man-made

I recently inherited my grandparents coin collection, mostly from 1920's to 1970's.
I know nothing about coins and this site has been a great place for research.
The collection is nice, all circulated, but there's one nickel that is out of place.
1920's nickel with 5 punch holes over the date, error / factory damage or man-made?

Man made.
Known as PMD (Post mint damage) in Numismatist terms.
