1945 nickle has Indentation in center of coin makin it 3d-ish from other side of coin?
Do these kind of errors happen after they leave the mint? Or would this happen at the mint and be considered an error ? Thanks for any advice!

It would be PMD, that could not have happened during the striking of the coin.
Also an error cannot happen after the coin is struck.
